Branded vs Generic Semaglutide in India — Same Molecule, Very Different Price
Last reviewed 12 May 2026 · India-specific clinical evidence
The short answer
Branded semaglutide (Novo Nordisk’s Ozempic and Wegovy) and Indian generic semaglutide are the same active molecule. Since the patent expired in March 2026, DCGI-approved generics — Sundae, Semaglyn, Semasize, Noveltreat and others — deliver identical semaglutide from about ₹220 per weekly shot, 3–5× cheaper than branded. Both need a prescription. Branded still leads on the full 2.4 mg obesity dose and pen polish; generics win decisively on price.

Efficacy — what the trials show
Branded Semaglutide
Branded semaglutide carries the original evidence base: the SUSTAIN trials (diabetes) and the STEP trials (obesity). STEP-1 (NEJM 2021) showed a mean 14.9% body-weight reduction over 68 weeks at 2.4 mg; SELECT showed a 20% reduction in major cardiovascular events in people with established CVD and obesity. This is the data package regulators and doctors reference.
Generic Semaglutide
Generic semaglutide is the same molecule, approved by DCGI on bioequivalence — meaning it is required to behave the same in the body within a tight statistical margin, so clinical efficacy is expected to match branded at the same dose. Several Indian makers (e.g. Sun Pharma, Alkem) also ran India-based Phase III bridging studies. The main practical caveat is dose ceiling: many generics stop at 2 mg rather than the 2.4 mg obesity dose.
Cost in India (2026)
Branded Semaglutide
Branded runs ₹5,660–9,100/month for Ozempic and ₹5,660–16,400/month for Wegovy (scaling with dose), even after the April 2026 price cuts. The 2.4 mg Wegovy maintenance dose sits near the top of that range.
Generic Semaglutide
Generics start at about ₹220 per weekly shot — roughly ₹1,290/month for the cheapest vial brand (Eris Sundae) — and most pen generics land between ₹2,200 and ₹8,000/month. For a therapy taken for a year or more, that is a very large difference for the identical molecule.
Side effects
Branded Semaglutide
Because it is the same molecule, the side-effect profile is the semaglutide profile: nausea, diarrhoea, vomiting, constipation and abdominal discomfort, peaking during dose escalation and easing at a stable dose. Rare but important: pancreatitis, gallbladder events, and worsening of unstable diabetic retinopathy.
Generic Semaglutide
Identical molecule, so the side-effect profile is the same as branded at the same dose. Any real-world difference tends to come from pen ergonomics or cold-chain handling between manufacturers, not the drug itself — which is why buying DCGI-approved brands from licensed pharmacies matters.
Which one should you consider?
Pick Branded Semaglutide if
- ✓You need the full 2.4 mg obesity maintenance dose (Wegovy)
- ✓You want the exact product studied in the trials
- ✓You value established pen design and cold-chain consistency
- ✓You’re documenting therapy for insurance or workplace wellness
Pick Generic Semaglutide if
- ✓Cost is a major factor (generics are 3–5× cheaper)
- ✓You have T2D or are in early obesity titration (≤ 2 mg)
- ✓You want a vial or reusable-pen option, or an oral tablet
- ✓Your doctor is comfortable prescribing a DCGI-approved generic
